Public Records Request

Public Records may be obtained by submitting the Public Records Request Form to City Clerk Melissa Honeycutt.

Complete the form attached and submit it one of the three ways:

  1. Email it to cityclerk@cityofgardendale.com OR
  2. Mail it to PO Box 889, Gardendale, AL 35071.   OR
  3. Submit in person City Hall, Administration counter at 925 Main St., Gardendale, AL 35071
